SEAMAN CARD
For Local Seafarer
- Enclose a copy of NRIC
- Hold a valid Medical Examination Certificate
- In case lost of Seaman Card / Discharge Book, A copy of the police report needs to be enclosed
Any Malaysian who wish to register, should satisfied one of these requirement:-
- Have an experience of sea service on board merchant vessel, by possessing a 'Discharge Certificate', guarantee letter
from shipping company or other relevant proved, or
- Have gone through the training programs at Akademi Laut Malaysia (ALAM), or other relevant institution, or
- Have an experience of sea service with the Malaysian Royal Navy (who wish to joint the merchant fleet), or
- Holder of any Certificate of Competency approved by the Marine Department.
For Foreign Seafarer
A person who want to register has to fulfill the following conditions :-
- While serving on board Malaysian ship or has been offered to serve on board Malaysia ship;
- Holding a valid Medical Examination Certificate
- In case of lost of Seaman Card, a copy of the police report needs to be enclosed
FEES
- New and Renewal of Registration of Local Seafarer RM20.00
- New and Renewal of Registration of Foreign Seafarer RM50.00
- Lost of Card - Local Seafarer RM40.00; Foreign Seafarer RM100.00
- Seafarer Discharge Book RM30.00
- Lost of Seafarer Discharge Book RM60.00
Payment through money order must be made under the name of: PENGARAH LAUT WILAYAH
FORMS
Application of the Seaman Card have to be applied by using:
Form JL/HEPP/B/01 PIN.1/99 (Seafarer Registration Form) which provided free of charge
