SEAMAN CARD


For Local Seafarer
  1. Enclose a copy of NRIC
  2. Hold a valid Medical Examination Certificate
  3. In case lost of Seaman Card / Discharge Book, A copy of the police report needs to be enclosed
Any Malaysian who wish to register, should satisfied one of these requirement:-
  1. Have an experience of sea service on board merchant vessel, by possessing a 'Discharge Certificate', guarantee letter from shipping company or other relevant proved, or
  2. Have gone through the training programs at Akademi Laut Malaysia (ALAM), or other relevant institution, or
  3. Have an experience of sea service with the Malaysian Royal Navy (who wish to joint the merchant fleet), or
  4. Holder of any Certificate of Competency approved by the Marine Department.

For Foreign Seafarer

A person who want to register has to fulfill the following conditions :-
  1. While serving on board Malaysian ship or has been offered to serve on board Malaysia ship;
  2. Holding a valid Medical Examination Certificate
  3. In case of lost of Seaman Card, a copy of the police report needs to be enclosed

FEES
  1. New and Renewal of Registration of Local Seafarer RM20.00
  2. New and Renewal of Registration of Foreign Seafarer RM50.00
  3. Lost of Card - Local Seafarer RM40.00; Foreign Seafarer RM100.00
  4. Seafarer Discharge Book RM30.00
  5. Lost of Seafarer Discharge Book RM60.00
Payment through money order must be made under the name of: PENGARAH LAUT WILAYAH

Form JL/HEPP/B/01 PIN.1/99 (Seafarer Registration Form) which provided free of charge
